Between July 1996 and May 2000, 123 patients were operated for rhinoplasty in otorhinolaryngology clinic. 68.3% (n=84) were operated by the closed technique and 31.7% (n=39) were operated by the open technique. Open technique was preferred by the complicated refinement of the nasal tip, secondary revision surgery, when trauma has disrupted the nasal tissue and made it difficult to differentiate, where use of grafts are needed and for the education of trainee surgeons. Endonasal approach is preferred in simple and limited cases and by the experienced surgeons. Techniques for the approach to the nose is only the beginning of the surgery. The surgeon has to choose the technique, that he can manage and prefer. This technique must provide the best exposure, the best way of management and the least complication. Therefore refusal of a technique and supporting another is not an objective behavior for a surgeon, who is a scientist as well. External approach provides new opportunities and safety for the inexperienced surgeon. If there is indication and if the way of management with the complications is known, it can be chosen. Open technique is also important in a otolaryngology clinic, because it helps the education of the candidate surgeons. |
